An unplanned hospital stay can bring a whirlwind of questions, decisions, and emotions—especially when it comes to the next steps. Join us for an informative and compassionate conversation designed to help you navigate the post-hospital journey with clarity and confidence.

Our expert panel will walk you through essential considerations following a hospital discharge, including:
• Understanding your needs and available options for care
• Preparing important legal documents before a crisis occurs
• Exploring the differences between senior living options—Independent Living, Assisted Living, Memory Care, and Skilled Nursing
• Knowing how and when to start the conversation with your loved ones

Whether you’re planning ahead or responding to a recent event, this discussion will offer valuable insights and practical guidance to help you make informed decisions for yourself or a loved one.

Join us to gain peace of mind, ask questions, and connect with a community that’s here to support you every step of the way.
